The study is broken and cannot be completed. It links to a Google Form that you need access to. Even if it's fixed, don't take it. Prolific studies that are permitted to collect personal information have a specific Prolific website warning on them that is on their study task page (where you accept a study- same place as its description, pay, etc) to clarify that it is verified by Prolific. This study doesn't have that and is likely an identity theft attempt.
